According to the AACN, the DNP must possess skills for inter-professional collaboration for improving patient and population health (Zaccagnini, 2016). Collaboration is defined as working with someone to produce or create something (at-risk-Webster, n.d.). The DNP is fully prepared to collaborate with other healthcare members. A specific example is in population health. There are numerous health care initiatives that focus not only on treatment of conditions but identification of at risk populations. Health care initiatives also work in the prevention of illness and disease. The health care environment is complex. Contributions need to be made by highly skilled individuals across multiple disciplines. This improves the patient likelihood of experiencing a high-quality outcome because each Merriam brings a unique skill set to the table.
The Healthy People 2020 Heart Disease and Stroke have an objective to increase patients with controlled hypertension by 60%. An initiative that demonstrates collaboration across disciplines is the “Million Hearts” hypertension control program. The program was started Numerous health care initiatives focus years ago and the with a variety of disciplines to prevent 1 million heart attacks and strokes over the five years. It is co-led by the CDC and CMS (ODPHP, 2016). It aligns, coordinates and advances activities within communities using diverse collaboration. This is an excellent place the DNP can participate in team leadership. The DNP has the knowledge to identify barriers or gateways in healthcare to a variety of patient populations within a variety of settings.
