Physical Examination
Always plot the weight, height, and BMI on a growth chart. Measure blood pressure. You are looking for trends. Do not just look at the numbers obtained during the visit. How do they compare to the last well visit? Last well visit the weight was at the 25th percentile but this time it is at the 75th percentile….what happened?

Skin
- Examine for signs of abuse or self-injury, such as cutting.
- Observe for any nevi or birthmarks.
- Document tattoos and piercings.
Mouth
- Inspect for caries, gingival inflammation, or malocclusion.
Breasts and genitalia
- Assess for sexual maturity rating.
Heart
- Auscultate for murmurs (Often heard during this time ).
- Monitor blood pressure annually.
Musculoskeletal
- Observe hip, knee, and ankle function and gait.
