There are various tools you can use when assessing the teen’s life to identify problem areas. The tools are designed using open-ended questions about home, education, activities, drug use and depression, sexuality, suicide, and safety.

The HEADS mnemonic is widely used to help remember the important aspects of the psychosocial history. HEADS moves from the least to the most personal areas while building rapport.
