National Childhood Vaccine Injury Act

This act requires healthcare providers provide parents or patients with copies of Vacciine Information Statement (VIS) before administering each dose of the vaccine (according to recommended schedule). Additional information is available at localhealth departments and from the CDC.

Provider Documentation

After giving vaccine, document the following:

  • Name of vaccine
  • Date given
  • Vaccine information sheet date
  • Manufacturer
  • Lot number
  • Dose
  • Site
  • Signature & title of person giving vaccine
ACIP Guidelines
Doses may be given 4 days prior/later than specified date
Give live virus vaccines at least 28 days apart
Space vaccine injections 1″ apart; no need to aspirate
Reimmunization is not harmful
Reduced doses should not be given
Recognize/respond to adverse reactions
Vaccine failure can occur with improper transport and storage
May have accelerated schedule for travel
Major contradication is anaphylaxis

Immunization Schedule

  • Immunization schedule is revised at least annually
  • NPs must monitor American Academy of Pediatrics (AAP) & CDC websites for periodic changes
  • Refer to chart for patients ages 0-18 years of age and adult schedules
  • There is also a catch-up schedule for children at the CDC site
