Interval History

What happened since the last health supervision visit?   Be sure to include any parent concerns.

Examples

  • “What are you most proud of since our last visit?”
  • “What do you like most about your son/daughter?”
  • “What questions or concerns do you have about your child?”

Ask about past history.   Any visits to the emergency department or urgent care?   Any changes in the health of anyone in the family?

Developmental Red Flags (Warning Signs)

The child is unable to:

  • hold a crayon with a fist.
  • give a full name.
  • dress self.
  • tell a story.
  • balance on 1 foot for 1-second.

Developmental delays may not be noticeable earlier but become more obvious as the child develops.   Often the parents are the first ones to notice problem (e.g. “something is not right” or “my other child was doing this by now”).   The signs may point to motor problems (e.g. cerebral palsy), and/or cognitive problems.

Ask the child:

  • his/her name
  • age

 

Language Development and Self-help Motor Development
Does your child

  • Recognize colors?
  • Tell a simple story from a book or television?
  • Use some 3-4 word sentences?
Gross Motor

  • Jump with both feet?
  • Kick ball with direction?
  • Pedal a tricycle?

Fine Motor

  • Draw a person with head and 1 other body part?
  • Imitate drawing a cross? (Have them draw a picture while you talk to their parent.)
  • Put on shoes? (Have them put their shoes on after the PE.). Have them draw a picture while you talk to their parent.   Ask them to put on their shoes after the physical exam.
