I found your poster very intriguing as I work rural health we often don’t get to have our patients use care management as much. We only just recently with the virus started using Telehealth. Your information is something I would like to be able to share with my boss to see if we could use some of it to try to implement something? Thanks for your research.
I work for a Magnet organization. The organization is always innovative. The organization is able to see that investing in patients health and prevention is cost effective in the long run. Care Managers cannot bill for their services at least in the clinic but the reduction in cost from decreased ED and hospital admissions makes it cost effective. My clinic was the last one of our organization to role out the new primary care model with care management and social workers in the clinic. Our providers and patients love it. I have had patients tell me it is so difficult to get in touch with their specialty providers. I am able to be their advocate and make that contact and bridge gaps between patients and providers. I love the patient education aspect of the role and the relationships that are built.
Since the pandemic we have been able to utilize video visits with our patients. The majority of patients have enjoyed getting to see a friendly face instead of just the phone call. Especially during this time when we have not been able to meet in person yet. I also have a couple of patients with new trachs so utilizing MyChart messaging has been helpful. I would love to see more utilization of this technology. I think that is one positive out of this pandemic the medical field will continue to utilize technology to reach more patients.
I had a difficult time finding articles that were specific to care or case management. I would be happy to further discuss the role with you.
Your poster was pleasing to the eye. Graphics looked great. I have only worked in the inpatient setting during my nursing career and I know very little about the outpatient side. I know every patient in my facility is assigned a case manager during their stay. After discharge the case manager call and check up patients one week to one month after hospital stay. On one occasion, a case manager even found a window AC unit for a patient trying to heal from a wound in the midst of summer with no proper cooling system in their house. As an inpatient nurse, I don’t know what we or our patients would do without our case managers.
Thank you! I am very fortunate to also have a social worker in our clinic. We work together as a team for many patients. I really enjoy this current role. I am able to help patients in a variety of ways. We also get to know them a little more to identify daily needs that the patient may have.
Great poster! I think this can really help improve patient outcomes. Luckily many people are becoming more comfortable with digital devices. I think as long as the information stays simple and methods are user friendly then patients will not feel intimidated using these different channels.
